I / we are the owner/tenant (can we have a drop down for them to choose between owner or tenant) of Unit (they have to fill in unit number) and we wish to apply for Body Corporate approval to keep a pet as outlined below.

I / we agree to the following Body Corporate conditions for pet approval:

• To abide by the Body Corporate by-laws at all times;
• The pet is kept within the lot;
• The pet does not cause a nuisance to or disturb any other lot owners or occupiers;
• The pet is prohibited from any common area facilities (eg pool, spa, lounge, tennis court);
• The pet is restrained at all times whilst on common property and is carried or kept in a pet trolley whilst in entry lobbies, individual floor lobbies and lifts;
• The pet is domesticated, kept clean, quiet and controlled at all times, whilst inside the lot or anywhere on common property;
• The pet is registered and vaccinated; 
• The pet does not go to the toilet or run loose anywhere on common property.  If the pet has been found to make any mess on common property the owner of the pet accepts full liability for the costs of cleaning and making good that area;
• The pet wears an identification tag clearly showing the owners name, address and telephone number;
• The owner of the pet indemnifies the Body Corporate from any claim of accident or injury that may be caused by or in relation to the pet;
• The unit is appropriately fumigated on a regular basis to ensure pests and vermin are not present in the lot at any time;
• If any complaints are received in relation to the pet the Committee may review its approval and order that the pet be removed from the scheme land immediately.
